Instructions for the Post-Graduate Application
Welcome to the online application for post-graduate programs in the Eastman Institute for Oral Health
at the University of Rochester. An application requires:
• Online Application
• Three letters of recommendation (minimum)
• Official transcript(s)
• WES Evaluation for transcripts from foreign institutions
• Curriculum Vitae/Resume
• Personal Statement
• 2x2 Photo
• Dental School Diploma (copy)
• Official Standardized Test Report(s)
• National Board Exam Scores, Part 1 (required for applicants to GME residency programs)
• Signed Certification Statement (part of online application)
• Payment of $195.00 application fee is required at application submission
Application
Upon creation of an account, a confirmation email will be sent to the email address
used to create the account. This email address will serve as your login in the future.
The online application consists of 14 sections pertaining to personal information,
education background, and program objectives.

Letters of Recommendation
Each program requires at least three letters of recommendation, preferably from members of the
academic community who can speak to your potential as a post-graduate student. If a recent dental
school graduate and/or expected to graduate this upcoming year one of the reference letters must be
from your dental school dean. You are required to provide the names and contact information of your
recommenders in the online application; they will be contacted via email and asked to upload their
letter of recommendation. We encourage you to contact your references in advance, to ensure they
expect our request for a recommendation and will provide a submission to support your application.
You will be able to view the status of the electronic submission of your references by returning to
the online application, and you will have the ability to re-send the email request if necessary by clicking
on ‘Status Details’.
Transcripts
Official transcript(s) must be sent directly from your dental school and, if applicable, graduate institutions
to the address shown in the mailing address listed below. Transcripts must be in English or submitted with an English
translation provided by a qualified U.S. based translation service.
WES International Credential Advantage Package (ICAP)
Transcripts from foreign-trained applicants must be evaluated and submitted by World Education Service (WES).
We do not accept these evaluations directly from the applicant. We do not accept evaluations from
Educational Credential Evaluators (ECE).
Personal Statement
Every program requires a personal statement that describes your academic and/or professional background, research,
or clinical experience, career goals, reasons for entering your chosen field, and reasons for selecting the
Eastman Institute of Oral Health at the University of Rochester for advanced clinical training.
Standardized Tests
Applicants are urged to take required tests so that the results can be considered in conjunction with
the candidate's academic records.
Test of English as a Foreign Language (TOEFL)taken within a 2-year period of the application date,
is required of all applicants whose native language is not English. U.S. citizens and/or permanent residents
are exempt from the TOEFL requirement. School code is 9487. We do not accept IELTS.
Graduate Record Examination (GRE) is required for consideration for the Master of Science
in Dental Science programs. School code is 9487.
Dental School Diploma
Applicants trained in the U.S. or Canada must upload a copy of their dental school diploma. An original
diploma is required if admission is offered and accepted. Original copy of the diploma must be presented
upon matriculation.
Foreign-trained applicants must submit a certified evaluation of the diploma completed
and submitted directly by World Education Service (WES). We do not accept evaluations from Educational Credential
Evaluators (ECE).
Supplementary Information for Non-US Students
Documents or letters submitted in support of your application must be written in English or be
accompanied by an English translation provided by a U.S. translation service. Please do not send secondary
school records. In many countries visas will not be issued without proof of adequate
proficiency in English (such as a satisfactory TOEFL score). To obtain a visa for study in
the U.S., international students must present the Certificate of Eligibility (I-20 or DS-2019)
at a US consulate or embassy and demonstrate proof of ability to pay all tuition
and living expenses for the first year of study.
The University of Rochester International Services Office issues the
I-20 or DS-2019 after a student has accepted the admission offer and has demonstrated
proof of his/her ability to pay all expenses for the first year of study.
